Linden Grove

Our program provides care for patients residing at the Linden Grove Communities in Waukesha. This facility houses assisted living, memory care, and subacute rehabilitation. Nestled in a quiet residential neighborhood less than ten minutes away from the Waukesha Family Medicine Center, this 150-bed facility provides our residents with a convenient setting to learn about caring for older adults in conjunction with a team of highly skilled health professionals including nursing staff, physical and occupational therapists, dietitians, and social workers.

Residents and faculty maintain a weekly clinic in the assisted living facility to provide convenient on-site care for patients residing at the facility. We are currently expanding our services to also provide services to patients in the memory care facility. Residents on the geriatric rotation have the additional opportunity to provide care for patients in the subacute rehabilitation unit.
